Levels of Amputation

Partial foot prostheses include several different levels of amputations, as follows:

  • Chopart Amputations- these are transtarsal amputations that preserve the talus and calcaneus. The Chopart joint, or the transverse tarsal joint, and an amputation at this point preserves plantarflexors, but sacrifices the dorsiflexors.
  • Lisfranc amputations- this is when the amputation removex the metatarsals, while maintaining the plantarflexors and dorsiflexors muscles and their attachments
  • Transmetatarsal amputations (TMA)-these remove all or part of the forefoot, including the metatarsal bones, or the five long bones between the toes and ankles.
  • Toe amputations- commonly of the greater toe or the second toe, can be full or partial removals of the toes. Amputation of the greater toe causes loss in biomechanical function of foot during the gait, such as propulsion and balance.

There are several different approaches to treating a partial foot amputation, depending on the level. You can do no prosthesis (toes other than the great toe), custom foot orthotic with toe filler (for the great toe amputation), custom toe filler with rigid carbon fiber foot plate (for TMA’s), modified ankle-foot orthosis with a toe filler (Lisfranc amputation)and a foot prosthesis, which is a custom-molded socket attached to a semi-rigid foot plate with a toe filler (Chopart Amputation).

Prosthetic Feet

There are many different types of feet to choose from in the market today. When you are coming up with a plan for your prosthesis, your practitioner will ask you about your lifestyle and activities you participate in. This will include how far you walk, what sport(s) you play, are you on uneven terrain for camping, hiking, hunting, and how often you are on your feet. All these different activities you do every day play a crucial role in deciding what foot will be best for you. Each foot offers different benefits to different activities from having energy return in a Rush 87 foot to a Microprocessor Otto Bock Smart Ankle that can plantarflex and dorsiflex like your foot during your gait. The different types of feet are as follows:

Solid ankle cushion heel (SACH)

The SACH foot is the most basic prosthetic foot, and one of the lowest cost feet available. It includes a solid ankle and rigid keel that runs along the sole of the foot. The heel is cushioned by a foam wedge. This foot is often used on the initial prosthesis.

Flexible Keel

The flexible keel foot is foot the accommodates for ground reaction forces when ambulating. little more flexible than SACH feet. They allow the front part of the foot to adjust to varied walking conditions but stay stiff and stable while standing or walking.

Single Axis

The single axis has bumpers, which control ankle flexion and allows the forefoot to quickly contact the ground following heel strike. This foot is a bit heavier than more basic feet, like the SACH.

Multi-Axial

The multiaxial foot has multiple components at the ankle that allows for maximum amount of motion. The multiaxial foot will conform to a variety ofuneven surfaces and move from side to side.

Dynamic Response

Dynamic response feet flex at heel strike and midstance and spring back at late stance, which mimics the energy responses of a normal ankle to reduce energy expenditure and improve gait functionality.

Multi-axial Dynamic Response

This foot type is a combination of the dynamic response and multiaxial system, allowing for maximum range of motion in the ankle with a dynamic response from heel strike to late stance. This ankle/foot system is recommended for the moderately-extremely active patient.

Range of Motion

The ROM foot allows for natural range of ankle movements. Its natural motion adjusts to slopes, various heel heights, and sitting with flat feet. Because it is so adjustable, the foot also comes with a manual lock to prevent unwanted movement in certain situations.

Microprocessor

Microprocessor feet can include electronic foot and ankle units which possess artificial intelligence to determine the walking pattern of the user and adjust the ankle position to the appropriate amount of flexion through self calibration.
